Skilled nursing is a term that refers to patient's need for care or treatment that can only be done by licensed nurses to address a medical condition.  Our team of Registered Nurses and Licensed Vocational Nurses go above and beyond to provide the best possible health care in the comfort of our patient's home. Our Skilled Nurses coordinate the patient's plan of care, treat the condition, monitor progress and help the patient remain independent at home.  Our skilled nursing services are inclusive of, but not limited to the following:

Medical Social Work

Medical social workers evaluate social and emotional factors that affect your health and wellness. These individuals help you and your family manage stress and connect you with available community resources. Medical social work is delivered in accordance with your doctor's orders and as defined in a plan of care. Medical social workers provide:

  • Community resource planning

  • Referrals

Speech Therapy

Speech and language pathologists help you restore speech that may have been lost due to some form of trauma, as well as train you to improve breathing and swallowing. Services may include:

  • Alternate communication methods

  • Voice and speech retraining

  • Esophageal speech training

  • Cognitive retraining

  • Oral/facial exercises

  • Swallowing disorder treatment

Home Health Aides

Home health aides help you manage daily living activities, and many are trained specially to deliver more complex services under the direction of a registered nurse.

Workers Comp

Workers’ compensation home care services provide assistance and support to individuals who got sick or injured due to a work-related injuries or illnesses and as a result are entitled to certain workers compensation benefits.  These services assist injured or ill workers with their recovery, rehabilitation, and daily living activities while they remain in their homes.

Our team works closely with case managers, claim managers, insurers, and state programs to ensure our clients get back in shape as fast as possible and receive all the necessary support and dedicated care.

Occupational Therapy

Occupational therapists help you perform daily living activities such as bathing, eating or dressing. Individuals with physical, developmental and/or social problems might need the skills of an occupational therapist. Services may include:

  • Helping you adapt equipment you use on a regular basis

Physical Therapy
